[dc401-l] Bowling Shirt Order Form

The bowling shirts are ready to be ordered.

As decided at the last meeting, it's going to be Che's Quahog and  
Crossbones design. The shirts will be coming from the aptly named  
bowlingshirt.com. They're black with white trim, 55/45 Cotton/Poly and  
the design will be screen printed on the back in white. A lot of the  
available shirts are pure synthetic, which I don't think anyone really  
wants so 55/45 was the best we could get.

Here's the order form. Fill it out and email it back to me. Come to  
the June meeting and pay me in cash or paypal the money to me as noted  
below.

You've got lots of options. Chain stitched name, embellishments. For  
the chain stitched names, I'll be instructing them to do them case- 
sensitive if possible, so fill that out exactly how you want it on the  
shirt.

All orders must be in and paid for by the June meeting. I'll be  
placing the order on June 5th.
